Robson returns from solitary to discover he's been kicked out of the "Brotherhood" and moved to another cell. McManus tells Glynn he'll write a letter supporting Alvarez's bid for parole. Problem is, Alvarez attacks one of the parole board members and gets thrown back into solitary. Dr. Nathan's guilt worsens as Chucky slips into a coma from the septic infection. Schillinger tells new inmate Cutler he has to kill a black inmate to join the "Brotherhood" and Cutler selects Said as his victim. White's gets this information and goes to protect Said only they get into a tussle and Cutler almost dies. Rebadow gets to go see his dying grandson only to have to return later for the boy's funeral. Busmalis gets a letter from Norma who wants to visit. She does and brings surprising news. Beecher moves to make a confession during the interaction session. The result? Schillinger gets convicted of rape and is transferred to solitary indefinitely. Father Meehan touches a nerve with Ryan and forces him to acknowledge a childhood incident that scarred Ryan for life. A stabbing intended for Redding, claims an innocent inmate instead.

Robson viene estromesso dalla fratellanza ariana per via delle sue gengive provenienti da un uomo di colore. La sua disperazione è tale che prova a tagliarsele via con un coltello improvvisato. Alvarez non regge la pressione dell'udienza per la libertà vigilata, e dopo aver picchiato uno dei membri che lo stava accusando duramente viene rispedito in isolamento per l'ennesima volta. Gloria sente di doversi prendere cura di Pancamo, che nel frattempo è finito in coma. Omar finisce quasi con l'uccidere uno degli ariani che vuole far fuori Said e McManus non può far altro che spedirlo in isolamento. Beecher decide di parlare e rivelare la verità sugli stupri di Winthrop e Guenzel e la morte di quest'ultimo: Schillinger viene accusato per i due stupri e trovato colpevole, finendo così in isolamento a tempo indeterminato. Keller viene condannato per uno dei vecchi omicidi alla pena capitale. Rebadow lascia OZ per la prima volta in 35 anni per poter assistere suo nipote, che morirà di li a poco. Cyril si rende conto che sta per morire e scatena la sua ira contro gli AC. In una conversazione col padre Ryan rivanga un brutto episodio del suo passato che ha a che vedere con la sorellina morta in tenera età. Morales rompe definitivamente con i neri e trova nuovamente alleanza con gli italiani, la prima mossa del nuovo accordo prevede la morte di Redding: il siciliano Frank Urbano si incarica di far fuori l'uomo, ma quando sta per colpirlo Augustus si mette in mezzo, salvandolo, ma ricevendo la coltellata mortale al posto di Burr.
Robson regressa da solitária e descobre que foi expulso da "Irmandade" e transferido para outra cela. McManus diz a Glynn que escreverá uma carta a apoiar a tentativa de liberdade condicional de Alvarez. Mas o problema é que Alvarez ataca um dos membros da comissão da condicional e é devolvido à solitária. O sentimento de culpa da Dra. Nathan piora quando Chucky entra em coma por causa da infeção sépticémica. Schillinger diz ao novo preso Cutler que tem de matar um preso negro para entrar para a "Irmandade" e Cutler seleciona Said como vítima. White obtém esta informação e vai proteger Said mas entra numa briga e Cutler quase morre.

Robson återvänder från isoleringen och har blivit utslängd från brödraskapet. Schillinger säger åt nya fången Cutler att han måste mörda en svart fånge för att vara med i brödraskapet, och Cutler väljer ut Said. Norma har överraskande nyheter åt Busmalis.
