Nach dem Kampf zwischen Kenshirô und dem Tyrannen Sanga kehrt immer noch kein Frieden im Land ein. Ein neuer Gegner taucht plötzlich auf – Seiji, der die Macht über das Land an sich reißen will. Kenshirô steht vor seiner bisher größten Herausforderung.
The Last Land has crumbled at the hands of Kenshiro, but in order to save the young Lord Doha, the powerful hero must travel through a land haunted by murderous demons in this sequel to the hit anime Fist of the North Star. Lord Doha has fallen ill, and if he doesn't receive a rare serum within a few short days he will certainly die. Though the serum is a two-day trip in both directions, Sara reveals that there is a shorter path running through Cliffland but it is haunted by vicious demons. As Kenshiro bravely makes way for the dreaded Cliffland, a new fighter named Seiji arrives in Last Land pursued by evil Clifflanders and vowing to reclaim Sanga's legacy.
Kenshiro quitte l'enclave de Lastland, à la recherche d'un remède pour Vista, grièvement blessé. Il doit pour cela traverser les Clifflands, un territoire que l'on dit hanté par des démons sanguinaires. Ces derniers, en réalité disciples d'une technique ancestrale appelée Hokumon-no-ken, se manifestent pour lui barrer la route. Au terme du combat, Kenshirô découvre le secret qu'ils cherchent à tout prix a dissimuler, et perçoit à nouveau le fardeau qui pèse sur les héritiers du Hokuto. Pendant son absence, un homme mystérieux, Seiji, arrive en ville. Consumé par une haine féroce, il vient pour tuer Sanga et prendre sa place sur le trône.
Neppure Sara riesce a guarire Vista che, ferito gravemente da Sanga e infettato dal tetano, rischia di spegnersi prematuramente: è necessario ottenere al più presto il vaccino per questa malattia, ma, per fare in tempo, si dovrà passare attraverso una montagna circondata da una leggenda secondo la quale nessuno di quelli che hanno tentato di superarla sarebbe mai tornato vivo. Kenshiro decide di partire immediatamente; contemporaneamente, però, Seiji giunge a Lastland e, grazie anche all'aiuto degli uomini del comandante Ches, riesce a impossessarsi della città, mosso da un profondo odio vero il padre, che lo abbandonò quando era ancora un ragazzo.
