Minakami Wataru hat zwölf kleine Problemchen, beim Namen Karen, Sakuya, Hinako, Marie, Chikage, Yotsuba, Haruka, Rinrin, Mamoru, Aria, Kaho und Shirayuki. Alles seine jüngeren Schwestern.
Mal davon abgesehen, dass er die Hochschule seiner Wahl nicht besuchen kann, da er den Einstellungstest verhauen hat und jetzt auf eine Schule, die auf einer merkwürdigen Insel steht, gehen muss, hat er jetzt auch noch mit zwölf Problemchen zu Hause und in der Schule zu tun.
Alle Zwölf sind einzigartige Charaktere, die eine ist ein Kochfreak, die andere ist in Sachen Technik unschlagbar, wiederum ist die andere nach Sport süchtig - doch alle haben sie eins gemeinsam: sie lieben ihren großen Bruder über alles.
Wataru Minakami is a top student who failed his high school entrence exam because of a computer glitch . He later discovered that he was accepted to Stargazer Hill Academy, which is located at a mysterious place called Promised Island. At the request of his father, Wataru is whisked away to the island, and before he can settle in, a dozen of cute and charming girls start to flock him and claim to be his younger sisters. As Wataru gets closer to his newfound siblings, a deeper mystery as to why they were sent to the island comes to play.
C'est l'histoire d'un jeune garçon qui aurait aimé être intégré dans un lycée très populaire, mais il n'a pas pu être accepté. Après être rentré chez lui, son manager lui dit que son père est parti. Des agents s'occupent de lui et l'amènent dans une île assez bizarre appelée l'Île Promise. Il rencontre là-bas des gens étranges et revoit régulièrement son manager qui lui assure qu'il existe plusieurs copies de la même personne à travers le monde. Au cours de sa journée, il aide plusieurs filles et se fait aussi aider par elles, mais ce n'est bien que plus tard qu'il découvre qu'elles sont ses propres sœurs. Or, le plus troublant, c'est que d'autres de ses sœurs apparaissent : il en découvre 13 dont une n'est pas sa sœur...
ある日突然妹が出来たら・・・。しかも、12人も。そんなシチュエーションから物語はスタートします。
主人公は兄(海神航)です。彼は、頭脳明晰の高校受験生(中学三年生)東京都在住。父は外交官で、母はいません。１人っ子で、小さい頃から父は外国へずっと赴任しており、実際は「じいや」が育ててくれました。そんな彼が高校受験をしますが、ちょっとしたミスで受験に失敗します。「じいや」も航が15歳までは面倒を見るように父から言われていましたが、中学卒業を機に航の元を去ります。途方にくれる航。身寄りもなく、高校浪人と思いきやそこへ高校入学許可を告げる黒服の男たちが登場。そしていきなりある場所へと連れて行かれます。いきなりの事件に戸惑いと混乱が入り乱れる航。そこに、さらに追い討ちをかける出来事が・・・。12人の女の子たちが次々に現れて、一斉に「お兄ちゃん」と航を呼びます。
所謂「妹妹」，其實是突顯主角「兄」（即讀者）的象徵性。在原作並沒有一個實際的「兄」的存在，大概是希望讀者能代入其中享受「妹妹」的愛之餘也能愛護「妹妹」。原作12個「妹妹」每一個都是獨立的存在。改編動畫和遊戲時才將「兄」實體化。在遊戲中玩家可選擇「妹妹」是否有血緣關係，每個妹妹均根據其誕生日的星座反映其性格，對哥哥的稱呼也都不盡相同。
고교 입시 합격자 발표를 기다리는 많은 학생들의 등장으로 시작된다. 주인공인 미나카미 와타루도 그중에 한명의 학생. 평소에 넘버원으로 통하던 그는 고교 입시 합격을 자신만만하게 기대하고 있었다. 하지만 시험 정답을 밀려 쓰는 바람에 고고 입시에 낙방 하게 된다. 이제 모든것이 끝이라고 생각하며 집으로 돌아 오는데 정체 불명의 남자 두명이 다짜고짜 와타루를 트럭에 태우고는 어디론가 가버린다. 정신을 차린 와타루가 와있는곳은 바로 프리미스 아일랜드 라고 하는 테마파크가 있는 한 섬이었다. 정체불명의 두명의 남자는 와타루에게 호시미가오 카니시 학원이라는 곳에 추천 입학했다는 말을 하고는 사라진다. 섬으로 간 와타루는 실수로 바다에 빠지고 당황하던 순간 한 여자아이에게 도움을 받고, 여기서부터 무려 12명이나 되는 여동생들에게 둘러싸이는 시스터 프린세스의 이야기가 시작된다.
Wataru Minakami es un estudiante destacado que reprobó su examen de ingreso a la escuela secundaria debido a una falla en la computadora. Más tarde descubrió que fue aceptado en la Academia Stargazer Hill, que se encuentra en un lugar misterioso llamado Isla Prometida. A petición de su padre, Wataru es llevado a la isla y, antes de que pueda instalarse, una docena de chicas lindas y encantadoras comienzan a reunirse con él y dicen ser sus hermanas menores. A medida que Wataru se acerca a sus nuevos hermanos, surge un misterio más profundo de por qué fueron enviados a la isla.
