南方仁（大沢たかお）が幕末の江戸時代に迷い込んでから、2年ほど経った。落ち込む橘咲（綾瀬はるか）の様子が気になり、咲の実家を訪ねた仁は、咲の兄・橘恭太郎（小出恵介）から、咲の母・栄（麻生祐未）が脚気（かっけ）であることを明かされる。何とかして栄を助けたいと考えた仁は、甘いものが好物だという栄のため、とあるお菓子をこしらえて脚気治療を行おうとする。
