子どもの頃から病のせいで家の中だけで過ごしてきた桜井萌。15歳になったある日、余命半年と医師から宣告される。家族が悲しみに暮れるなか、高校に通うことを決意した萌は、同じクラスの佐藤日向に突然の告白。少しずつ距離を縮めていく二人が迎えたのは、好きな人と一緒に見ると永遠に結ばれるという満月「ストロベリームーン」が見られる日であり、萌の誕生日である6月4日。そしてその日を境に、萌は日向の元から姿を消した。彼女の本当の想いは13年という時を経て日向の元へと届くのだった。
